[epicallusers] Effective September 05, 2024: New Bounce Back Readmission Risk Score replacing LACE+

The Bounce Back Risk Score tool has been developed under the direction of the Transitional Care Committee.

Effective September 05, 2024,  the Bounce Back Risk Score is an Epic tool used to more accurately predict the patient's likelihood of readmission.

The Bounce Back Risk Score will display in all areas of the chart that previously displayed the LACE+ Score, including the Summary Overview report, Navigators, Reporting Workbench Reports and Patient Lists.
