[epicproviders] Covid-19 Health Maintenance Topic

[Epic - All Provider Staff]

The Covid-19 Health Maintenance Topic will be assigned to all patients 12 years and older.  A Risk 3-dose series will be assigned to patients who have previously received a Pfizer or Moderna dose with designated Immunocompromised conditions or can be manually added via a Health Maintenance modifier.
