[epicproviders] Effective 8/31: Consult to Hospital at Home Order

Effective August 31st, 2022: The new Consult to Hospital at Home order has been added in Epic to help identify eligible Hospital at Home Patients. Once the order is placed, the Hospital at Home (HAH) team will evaluate the patient for eligibility. This is accordance to the Upstate Hospital at Home CM H-34<https://urldefense.com/v3/__https://upstate.ellucid.com/documents/view/11543__;!!GobTDDpD7A!LVoyXA3pFk4SZgiaaPFYD-kBS3Wi3xMydSny5_U4Rd_HuFoHO67F_3EB5VokighTre8OUi-sH-fAaZb1w7vA85RcCCr-x_hG$  > policy.
