[epicproviders] Effective May 11, 2022: COVID-19 Orders Update

Effective Wednesday May 11th, 2022:  The New York State Department of Health has determined that the Ask on Order Entry (AOE) questions within the COVID-19 orders are no longer required.  This change will decrease the amount of documentation required when ordering COVID-19 tests.
