[epicproviders] Effective 10/4/2023: Transition of Care to General Medicine BPA

Inpatient & Emergency Department Providers

Effective October 4, 2023:  To help improve the transition of care for general medicine patients, there is a new Epic Patient Safety BPA to update the patient's treatment team to either Hospital Team G: CC or MED: UHT 11. The BPA will populate based on the following documentation:

  *   Transfer: Cross Campus with Transport or Transport Patient (ED Only) order is placed and includes:
     *   Service: General Medicine or Hematology Oncology (transfer to UH only)
     *   Level of Care: Acute
     *   If the appropriate General Medicine team is not assigned:
        *   Hospital Team G: CC - patients transferred to Community.
        *   MED: UHT 11 - patients transferred to Downtown.
  *   Transfer: Same Campus with Bed Request order is placed and:
     *   Community Campus ICU Patients transferring to a medical floor without Hospital Team G: CC assigned.
     *   Service: General Medicine or Hematology Oncology (transfer to UH only)
     *   Level of Care: Acute
