</span><b>For pilot units</b> <b>UH 6A & 6B, UCH ICU</b> - patients identified as being at high risk for emergency department utilization or hospital readmission, an
<b>OurPractice Advisory (OPA)</b> will recommend a referral to the <b>Medication Adherence Program (MAP)</b>. The patient's MAP score is displayed in the advisory, and the referral order is selected by default.<o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">Once signed, the referral is added as a new discharge order, and program information is included on the patient's
<b>After Visit Summary (AVS)</b>. The <b>SUNY Outpatient Pharmacy</b> will then contact the patient to provide education and coordinate enrollment in the program.<o:p></o:p></p>
