Good afternoon,
It looks like these dates have already been blocked and no patients are scheduled. What else needs to be done?
Thank you,
Deosa

Subject: paid family leave
Good Afternoon and happy Friday, I was in touch with your colleague Katelynn Slater, a few weeks back about this and with my department coordinators. I’ve filled out the form (I think) for Paid family leave but understand I can’t submit until August 6 because of when I officially started. My plan is to start a vacation 8/15/2023 until 8/20/2023. Then I will hopefully be able to have my request fulfilled for FMLA/paternity leave/PFL from 8/21/2023 for 2 weeks ending 9/3/2023. Then I’ll take one more week of paid vacation starting 9/4/2023 ending 9/11/2023. Hope that makes sense. I’ll send the forms I have 8/6/2023 as instructed and hopefully it’ll go smoothly. With the imminent arrival of the new baby I’ve been a little scatter brained so appreciate your patience and your guidance. Thanks so much
Jason Sloane
