[JosProv-time] Time off

Since both of these dates are less then 30 day notice we will need to try and reschedule these patients to a sooner date.

There are currently 11 patients scheduled on 10/23 and 2 scheduled on 11/6. There are enough openings to try and reschedule those patients to the week of 10/16.

I would like to request off Monday November 6th if possible.

 Also, I know it's not a months notice but my daughter is having surgery on October 23rd, was wondering if I could get this day off as well?
