[JosProv-time] time off

Hi,
I unfortunately will be needing surgery. For this I will need to have time off for an appointment and for a week following the surgery. I will need to request off January 6th at 2:15-5pm and February 24-28th. I apologize for the late notice in this matter!
I had off February 20-21 for vacation but I am able to work the full day on February 20th and the morning of February 21st, and this may help with rescheduling some of the patients.
