[JosProv-time] Template updates

Hi all,

Please keep July 15 - 22 blocked as it already is, but put in the normal template so that I can start to add patients on this call week.

Also, I'd like to make a template change for the 2024-2025 school year, so these changes can be started on Sept 10, 2024. Hopefully there won't be too many changes required for scheduled patients this far in advance.

Monday
8 AM New Patient
8:45 Follow-up (Endo only, no DM)
9:30 New (Urgent Only)
10:15 Follow-up
