[JosProv-time] call switch

Good afternoon,

I was recently informed that I have an investigator meeting from June 16 - 19 to attend. Please block June 16 - 19 for me and move any patients scheduled those days to June 5th or June 8th.  Dr. Izquierdo is going to cover for me on call 6/17 - 6/22.

I will take his call from 6/3 - 6/8.  Please block 6/2 - 6/8 from any new patients, other than those being rescheduled from June 16 and 17th .
