[JosProv-time] Time off notification

Good morning,

I apologize for the later notice, but I have had some additional time-sensitive health appointments that have come up that I will need time off for. I will list them below.

September 25th - I will just need my 2:30 and 3:00 appointments blocked
October 6th - I will need the full day off

Again, I'm sorry I could not notify you about these sooner, but I appreciate your understanding and help with this.
