<p class="MsoNormal"><span style="color:black">I do not have a patient at 11am on December 24<sup>th</sup> and am requesting to block that slot for vacation. I will also be back in town on December 26<sup>th</sup> and can work the afternoon. Lastly, I currently
have vacation September 8-11th, which I believe is the first week of school for the kiddos, so I am requesting to switch my vacation time to the 15-18th, as it will be much less busy during this time. <o:p></o:p></span></p>
