<p class="MsoNormal"><span style="font-size:10.0pt;font-family:"Segoe UI",sans-serif">Medicine and laboratory testing are constantly changing. In an attempt to keep us all on the same page, the Laboratory Formulary Committee will be regularly sending out emails
with recent updates and guidances. Below are the short versions of relevant changes. More in-depth resources about the changes are attached.</span><o:p></o:p></p>
<p class="MsoNormal"><span style="font-size:10.0pt;font-family:"Segoe UI",sans-serif"> </span><o:p></o:p></p>
<p class="MsoNormal"><a name="_Hlk86927969"><b>Blood culture molecular panel:</b> Beginning
<b>December 6<sup>th</sup></b>, 2021, Microbiology will <u>automatically</u> perform a multiplex molecular assay on
<u>initial positive routine blood cultures</u> (<b>LAB462</b>) for <u>inpatients of all ages</u>; collection and ordering remain unchanged. This molecular panel, an updated version of the one currently in use for pediatric patients, detects multiple bacteria,
yeast, and antimicrobial resistance genes. <o:p></o:p></a></p>
<ul style="margin-top:0in" type="disc">
<li class="MsoListParagraph" style="margin-left:0in;mso-list:l0 level1 lfo1"><span style="mso-bookmark:_Hlk86927969">This supplemental analysis will initially be performed daily on all 3 shifts for patients ˜18 years and during the day shift only for adults;
24/7 testing for patients of all ages will be implemented as soon as feasible. <o:p>
</o:p></span></li><li class="MsoListParagraph" style="margin-left:0in;mso-list:l0 level1 lfo1"><span style="mso-bookmark:_Hlk86927969">For
<u>pediatric patients</u>: telephonic notification of the initial positive culture will include both Gram stain and molecular panel results.
<o:p></o:p></span></li><li class="MsoListParagraph" style="margin-left:0in;mso-list:l0 level1 lfo1"><span style="mso-bookmark:_Hlk86927969"><span style="mso-fareast-language:EN-US">For
<u>adult patients</u>: molecular panel results will be reported with the Gram stain report via telephone. If not available at the time of the Gram stain, molecular panel results will be available for review in Epic (panel results may require up to 18 hours
after the Gram stain positivity).<o:p></o:p></span></span></li></ul>
<p class="MsoNormal"><span style="mso-bookmark:_Hlk86927969">Standard identification and antimicrobial susceptibility testing of isolates will continue to be performed.
<o:p></o:p></span></p>
<p class="MsoNormal"><span style="mso-bookmark:_Hlk86927969">Infectious Diseases consultation is
<b><u>strongly recommended</u></b> when <i>S. aureus</i> (MRSA or MSSA) or yeast are detected.</span><o:p></o:p></p>
