The purpose of this form is to connect potential peer mentors with mentees in SUNY Upstate's MD-PhD that might benefit from academic guidance during their early years in the MD-PhD program. The responses from this form will be used to create a comprehensive
list of students willing to be peer mentors to fellow MD-PhD students. Signing up to be a peer mentor is a voluntary decision. However, becoming a peer mentor comes with some basic expectations of being available to mentor students that reach out to you to
the best of your capacity (depending on your and your mentees' schedules). Note that more than one student might reach out to you over the years, but you reserve the right to accept or decline multiple mentees, but should communicate that clearly to the student
so they may be able to reach out to someone else for mentorship.</div>
