The Well-Being Index (WBI) is an evidence-based metric tool developed by the Mayo Clinic to measure well-being. By your anonymous completion of the short (9 question) survey, you can get a snapshot of your own well-being at regular intervals, and also provide anonymous data to hospital and nursing leadership about the state of our collective well-being.

The QR code on the attached flyer provides a convenient link to the WBI survey – just use the password Upstate. The WBI also offers a very easy-to-use smartphone app for those who would rather complete it that way.

Last year, nearly 300 Upstate Nurses completed the WBI. This year so far, we have only captured data for about 60 nurses. As we build our wellness programs here at Upstate, your collective voice is important and we need to hear from more of you! Please consider taking a few minutes to complete the WBI or take a reassessment if it’s been a while.  Thank you!
