Cases to be discussed for Consultants’ Conference include:





1.        A 75-year-old man with multiple medical problems including hypertension, diabetes mellitus, hyperlipidemia, and sciatica presents to the ED for visual hallucinations over the past five days. He reportedly ran out of one of his pain medications one week ago. His initial vital signs are: BP, 100/56 mm Hg; HR, 66 beats/min; RR, 25 breaths/min; T, 36.9 degrees C; O2 Sat, 97% (RA).









2.        A healthy adolescent girl is admitted to the hospital for a “groin infection” that requires surgical intervention and intravenous antibiotics. On hospital day 7, the Poison Control Center is consulted because of an increase in serum creatinine to 8.64 mg/dL (reference range: 0.50 – 1.0 mg/dL) and a concern for “antibiotic toxicity.” Her vital signs at the time of consultation are: BP, 119/71 mm Hg; HR, 96 beats/min; RR, 22 breaths/min, T, 38.8 degrees C; O2 Sat, 100% (RA).











3.         A 44-year-old man presents to the hospital with “severe agitation” and requires pharmacologic sedation. He is given a 3-fold dosing error of a medication and is admitted to medical service for “acute mania.” On hospital day 2, his creatinine phosphokinase peaks at 13,000; on hospital day 5, he is cleared to the Psychiatry Service. Two weeks later, the Poison Control Center is called because the patient develops altered mental status and withdraws only to painful stimuli. Vital signs at this time are: BP, 124/75 mm Hg; HR, 114 beats/minute; RR, 18 breaths/minute; T, 100.2 degrees F; O2 Sat, 100% (RA).
