Cases to be discussed for Consultants' Conference include:





1.     A 2-year-old child presents to the ED for evaluation after biting into what appears to be a "fractured" thermometer. The child is asymptomatic and their initial vital signs are: HR, 113 beats/minute; RR, 28 breaths/minute; T, 36.1 degrees Celsius. The physician calls the Poison Control Center seeking advice for work-up and treatment recommendations.







2.     An 85-year-old man with a past medical history of paroxysmal atrial fibrillation, hypothyroidism, heart block with permanent pacemaker, and urothelial cell carcinoma s/p left sided nephroureterectomy two months ago, currently receiving immunotherapy presents to the ED complaining of generalized weakness and difficulty walking. His initial vital signs are: BP, 100/72 mm Hg; HR, 87 beats/minute; RR; 18 breaths/minute; T, 98 degrees Fahrenheit; O2 Sat, 98% (RA). His ECG shows a ventricular paced rhythm at 87 beats/minute and his initial high sensitivity serum troponin T is elevated to 1231 ng/L.







3.     A 62-year-old man with a past medical history of end stage renal disease receiving routine weekly hemodialysis presents to the ED complaining of dizziness, weakness, fatigue and feeling like he's going to "pass out." He recently had a new medication added started by his dentist to treat him for presumed trigeminal neuralgia. His initial vital signs are: BP, 189/104 mm Hg; HR, 90 beats/minute; RR, 20 breaths/minute; T, 97.9 degrees Fahrenheit; O2 Sat, 96% (RA).
