Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.

Cases to be discussed for Consultants’ Conference include:

1. A 13-year-old girl with a past medical history of major depression and prior history of suicidal ideation is found next to a half-empty pill bottle. She is non-verbal and her parents drive her to the hospital; en route, she has a witnessed generalized tonic-clonic seizure. Her initial vital signs in the ED are: BP, 146/84 mmHg; HR, 140 beats/minute; RR 37, breaths/minute; T, 97.7 degrees Fahrenheit; O2 Sat, 99% (RA)

2. A 30-year-old man with a history of polysubstance abuse presents to hospital with “intoxication” and is admitted to the hospital for “detox.” While boarding in the ED, he becomes agitated and is given a dose of 5mg intravenous haloperidol. Eight hours after ED arrival, he is found obtunded in his hospital bed and is intubated for airway protection. His post-intubation arterial blood gas shows: pH, 6.896, PCO2 38.9 mmHg, bicarbonate 7.5 mmol/L, and lactate > 24 mmol/L.

3. A 62-year-old woman with a past medical history of hypertension, hyperlipidemia, and diabetes mellitus presents to the ED with left-sided neurologic deficits that began approximately four hours ago; she is subsequently diagnosed with a cerebrovascular accident. While in the ED, she develops upper lip swelling approximately 1 hour after a medication is administered. She is otherwise in no acute distress. Her vital signs at this time are: BP, 121/73 mmHg; HR, 72 beats/minute; RR, 13 breaths/minute; T, 37 degrees Celsius; O2 Sat, 97% (RA).
