Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.

The next NYC Poison Center Grand Rounds and Consultants' Conference will be held on November 2, 2023. Grand Rounds will start at 1PM EST and Consultants' Conference will follow immediately. The topic and speaker will be:

Chemistry and How People Use Drugs
Adam Blumenberg, MD, MA
Assistant Professor of Emergency Medicine
Columbia University Medical Center

Consultants' Conference will take place from 2 PM - 4 PM EST.

The live conference will be held in the Public Health Laboratory and we will also broadcast via WebEx. Cases to be discussed for Consultants' Conference include:

1. A pair of 4-year-old twin girls present to the ED after ingesting their grandmother's medications. In the ED, they are found to be asymptomatic and with normal vital signs. Laboratory studies, including a serum concentration of one of the medications, are obtained. Based on the reported amount of medication ingested being considered "low-risk," the patients are discharged home without treatment. The next day, the mother reaches out to a toxicologist to determine if the treatment was appropriate and is immediately instructed to return to the ED.

2. A 66-year-old man with a past medical history of hypertension, diabetes mellitus, chronic back pain and alcohol use disorder is admitted to the hospital for an alcohol-related condition. After receiving treatment and having apparent improvement, he remains obtunded in the hospital for three weeks. An extensive workup for the persistent depressed mental status is unrevealing and the providers plan to transfer him to a comfort care facility. Prior to transfer, a medication is given and the patient returns to his baseline normal mental status. The poison center is consulted. Vital signs at the time of consultation are: BP, 110/80 mmHg; HR, 80 beats/minute; RR, 30 breaths/minute; T, 36.5 degrees Celsius; O2 Sat, 94% (2 l/min nasal canula oxygen).

3. A 17-year-old male with a history of viral hepatitis, presents to the hospital with altered mental status and after intentionally ingesting a substance he had collected over the past two weeks. Upon arrival to the ED, the patient is found to have a wide complex tachycardia and shortly afterwards has a ventricular fibrillation cardiac arrest. His vital signs on arrival to the ED are: BP, 110/60, mm/Hg; HR, 140s-200s beats/minute; RR, 20 breaths/min; T, 98.6 degrees Fahrenheit; O2 Sat, 100% on RA.
