Case discussion



Case #1: An 86 year old female presents to the emergency department after ingesting up to an entire bottle of their blood pressure medication. She is alert with vital signs including: blood pressure 136/92 mmHg; heart rate 72 beats per minute. Other vitals are presumed normal.

      Questions:

            1. What are the common blood pressure medications and which are considered more toxic in overdose?

            2. Anticipating a more toxic antihypertensive, what would be some considerations for treatment?

            3. Would you consider any laboratory studies important? Which ones?

                      4. Would you consider any empiric therapy should hypotension develop? Why

                      5. What if she also develops bradycardia?

Case #2: A 2 year old female is brought in BY EMS after a history of potentially ingesting 1-4 tablets of clonidine 0.1 mg. She has become progressively more lethargic from initial EMS call.

                     1. What would you like to know about her (additional assessments) at this time?

                     2. What would be an initial approach based upon the case information and answers to #1
