Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.
The next NYC Poison Center Grand Rounds and Consultants’ Conference will be held on January 4, 2024. Grand Rounds will start at 1PM EST and Consultants’ Conference will follow immediately. The topic and speaker will be:

Cases to be discussed for Consultants’ Conference include:

1. A 3-year-old boy with a past medical history of sickle cell disease is brought to the ED after he is found unresponsive by his father at home. In the ED, the child is lethargic and minimally responsive. When taking the medical history, the father reports that there is no known exposure to xenobiotics but upon repeat questioning, the father states that the child "could have been exposed to detergent.” The child’s initial vital signs are: BP, 60/20 mm/Hg; HR, 130 beats/minute; RR, 44 breaths/min; T, 95.0 degrees Fahrenheit; O2 Sat, 83% (RA).
2. A 30-year-old woman with no significant past medical history is found down in a bathroom at a homeless shelter. There is no apparent evidence of a drug overdose; she is not responsive to naloxone. Her vital signs on arrival to the ED are: BP, 157/86 mm Hg; HR, 140 beats/minute; RR, 25 breaths/minute; T, 101.2 degrees Fahrenheit; O2 Sat, 95% (Room Air). Her urine toxicology test is “positive” for cocaine.
3. A 78-year-old woman with a past medical history of atrial fibrillation presents to the ED for generalized weakness and nausea. She reports that she had been having increased palpitations for the past two weeks and she increased the frequency of her “atrial fibrillation medication” without consulting her cardiologist. Her initial vital signs are: BP, 103/68 mm Hg; HR, 56 beats/minute; RR, 22 breaths/minute; T, 97.6 degrees Fahrenheit; O2 Sat, 99% (RA). Her ECG shows sinus bradycardia, rate, 56 beats/minute; QRS duration, 250 msec; QTc duration > 600 msec.
