[epicproviders] Epigram: Update to COVID orders

COVID ordering has been updated to support new priority processing the lab can do. Instructions are displayed in the ordering panel when you select it.

Priority processing can be selected in the order for the following:

  *   ED patients likely to be admitted
  *   Patients transferred to inpatient from outside institutions
  *   Labor and Delivery patients
  *   Initial testing of symptomatic inpatients
  *   Homeless patients

For other patients, if you want to do priority processing, please contact ID before selecting the option. Approval is needed.
