[epicproviders] Adding Outside or Internal Infections from the Storyboard

When Care Everywhere is queried, Infection Status will now populate with outside infections, allowing Providers to add infections from an outside facility. There is also a new column available in Patient Lists that will allow you to quickly see which patients have outside infection documentation.
