[epicproviders] Updates to the Heparin Protocol

Inpatient Providers:

Effective Thursday October 15, 2020:  Based on end user feedback, updates have been made to the Heparin Infusion Anti-Xa / Weight Based Protocol order set.  Standing Anti-Xa orders have been removed and the Ordered Weight now appears in more places.
