[epicproviders] Effective 4/5/2022: Meds to Beds Auto-Enrollment for UH Heart Failure Patients

UH Inpatient Providers

Effective Tuesday April 5, 2022: Upstate University Hospital Heart Failure patients will be auto-enrolled in the Meds to Beds program based on problem list diagnosis of Heart Failure and an Ejection Fraction of 40 or less.  The patient's Meds to Beds pharmacy will default to SUNY Upstate Outpatient Pharmacy UH.

Heart Failure patients at Upstate Community General Hospital will not be auto-enrolled at this time.
