[epicproviders] Effective 4/4/2022: Comprehensive FRAIL Scale Assessment Note

Effective Monday April 4th, 2022: A new note template, SUNY Comprehensive FRAIL Scale, will be available. Providers can add the most recent Comprehensive FRAIL Scale Assessment into the body of the template by selecting the hyperlink "Comprehensive FRAIL Scale Assessment." The SmartPhrase .COMPREHENSIVEFRAILSCALE can also be used to insert the Comprehensive FRAIL Scale Assessment table directly into the note.
