[epicproviders] Effective 9/3/2025 - Updates to Medication Mixture Records

[Epic - All Provider Staff]

Effective September 3, 2025:  Inpatient Pharmacy will be moving all IV preparation into Epic.  As part of the conversion process, many of the mixture records will be replaced with new orders.  This may cause the removal of mixture orders from your preference list.  You will need to replace these orders on your personal preference lists and order panels.

Certain pharmacy IV preparations in Epic will calculate the drug volume, overfill volume, and add that amount to the base amount to create an "All In" volume.   This will be included in the total volume of the preparation, in the volume to be infused, and in the order name.


  *   "All In" allows for more accurate documentation of true volumes infused.
  *   The new volume of the medications will also integrate seamlessly with Alaris interoperability.
  *   The "All In" feature is used mainly for adult intermittent therapies.  Ex. infliximab, trastuzumab, pembrolizumab, leucovorin, etc.
