[epicproviders] Effective 9/8/2026 - Pilot Units 6A, 6B, and UCH ICU - OPA for Referral to Medication Adherence Program (MAP)

Effective September 8, 2026: For pilot units UH 6A & 6B, UCH ICU - patients identified as being at high risk for emergency department utilization or hospital readmission, an OurPractice Advisory (OPA) will recommend a referral to the Medication Adherence Program (MAP). The patient's MAP score is displayed in the advisory, and the referral order is selected by default.

Once signed, the referral is added as a new discharge order, and program information is included on the patient's After Visit Summary (AVS). The SUNY Outpatient Pharmacy will then contact the patient to provide education and coordinate enrollment in the program.
