The Upstate Trauma Department still has a limited number of leftover 2025 Trauma Shirts available for $10 each.
These shirts support Trauma Injury Prevention efforts, with all proceeds going toward education and programs aimed at reducing preventable injuries in our community. See attached image.

CAP Renewal or Level Advancement Application

  *   Deadline: 3/15/2026
  *   For Renewal:
     *   Submit an updated resume
     *   Include documentation of your CAP/NCAP goals achieved (e.g., meeting rosters, agendas, project summaries, PDSA cycles, research, or publications)
     *   For levels 6 & 7, include volunteerism.
