[reg] Issue with HealtheConnects form

Good Morning,

Front Desk end users are being asked to collect the HealtheConnections form again because the old one was taken off the Document List and the new one added this morning.  The helpdesk, Epic CPW, and Grand Central Trainers are aware. They just need to recollect it and it will store the form in Epic and won't be asked again.
