 NYC PCC Grand Rounds and Consultants' Conference in March 2021.


Our grand rounds speaker will be Konstantin Ichtchenko and the title of his talk is: “Neuronal Delivery Of Therapeutic Antibodies For Treatment Of Botulism.”

Bio: Dr. Ichtchenko scientific interests are centered on understanding the molecular mechanisms controlling exocytosis and endocytosis at nerve termini, and in designing molecular tools that enable the individual steps and components mediating these events to be dissected. He is the Principal Investigator on the funded $7 million NIH/NIAID multi-center grant supporting research by developing atoxic Botulinum Neurotoxin (BoNT) derivatives that retain the targeting and intracellular trafficking of wild type BoNT, thereby enabling delivery of the atoxic BoNT light chain (LC) to the neuronal cytosol. Dr. Ichtchenko is a widely acknowledged expert on protein engineering, and the molecular mechanisms of inter-neuronal and neuromuscular communication. He was responsible for the first successful expression and crystallographic characterization of the LDL receptor extracellular domain, done in collaboration with three Nobel laureates. Dr. Ichtchenko has also made major contributions to the discovery of neuroligins, molecules that are involved in synaptogenesis, and with yet another Nobel laureate, to the pre- and post-synaptic mechanisms of action of alpha-latrotoxin (black widow spider toxin). He is currently Assistant Professor in the Department of Biochemistry and Molecular Pharmacology, NYU Grossman School of Medicine.

Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.



The next NYC PCC Grand Rounds and Consultants’ Conference will be held on Thursday, March 4, 2021. Grand Rounds will occur from 1:00 to 1:45 p.m. EST. The topic and speaker will be:

Cases to be discussed for Consultants’ Conference include:





1.        A 35-year-old male patient with no known significant past medical history presents to the ED complaining of palpitations after using an illicit substance. He is admitted to the hospital for persistent altered mental status and a NSTEMI. Three days after hospital admission, he is noted to have a flat affect along with increased tone; a medical toxicology consultation is requested by the medical team. His vital signs at this time are: BP, 120/84; HR, 108 beats/min; RR, 18 breaths/minute; T, 35.6°C; O2 Sat, 99% (RA).







2.        A 6-year-old child with prior simple febrile seizures presents to the ED for altered mental status after a suspected ingestion of a product being used as a sleep aid. In the ED, the patient is noted to have a 3-minute “seizure-like” event with rigid extremities and drooling, followed by a brief period of unresponsiveness. The child’s vital signs after the seizure are: BP, 93/45 mm Hg; HR, 97 beats/minute; RR, 13 breaths/minute; T, 37.6°C, O2 Sat, 98% (room air).





3.        An 18-year-old female patient presents to the ED with altered mental status after an intentional suicidal overdose of her own medications. Her vital signs on presentation are: BP, 156/74 mm Hg; HR, 104 beats/minute; RR, 19 breaths/minute; T, 37.1°C; Os Sat, 100% (room air). Approximately 24 hours later, she is noted to have a transient non-sustained wide complex tachycardia seen on telemetry monitoring.
