Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.

The next NYC PCC Grand Rounds and Consultants' Conference will be held on September 1, 2022. Grand Rounds will start at 1PM EST and Consultants' Conference will follow immediately. The topic and speaker will be:

Cases to be discussed for Consultants' Conference include:

1.     A previously healthy 2-year-old child presents to the ED with multiple generalized tonic-clonic  seizures and a subsequent PEA cardiac arrest. Return of spontaneous circulation is achieved after resuscitation and intubation in the field. His post ROSC vital signs in the ED are as follows: BP, 92/41 mmHg; HR, 104 beats minute; RR, intubated; T, 99.1 degrees Fahrenheit; O2 Sat, 100% (on 40% FiO2), Point of care glucose, 104 mg/dL. A chest radiograph shows flecks of radiopaque material throughout his abdomen.

2.     A 40-year-old woman with no significant past medical history presents to the ED complaining of body-aches and tongue numbness after drinking an "herbal tea" she prepared to treat her father's "rheumatic pain." Her initial vital signs are: BP, 84/55 mmHg; HR, 60 beats/minute; RR 18, breaths/minute; T, 99 degrees Fahrenheit; O2 Sat, 100% (RA).

3.     A 37-year-old man with a history of seizure disorder presents to the ED with an elevated blood concentration of phenytoin. The patient was given a fosphenytoin load of 1000mg PE intravenously one day earlier after his neurology clinic found his blood phenytoin concentration to be subtherapeutic from blood obtained 3.5 weeks ago. The phenytoin blood concentration was 45.8 mcg/mL 2 hours after fosphenytoin administration and when repeated 19 hours later, was 44 mcg/mL. The patient is currently asymptomatic; the PCC is consulted regarding the approach to management. His vital signs at the time are: BP, 131/84 mm/Hg; HR, 76 beats/minute; RR, 17  breaths/minute; T, 97.9 degrees Fahrenheit; O2 Sat, 98% (RA).
