Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.



The next NYC Poison Center Grand Rounds and Consultants' Conference will be held on May 4, 2023. Grand Rounds will start at 1PM EST and Consultants' Conference will follow immediately. The topic and speaker will be:

Cases to be discussed for Consultants' Conference include:





1.     An 18-year-old man with no reported past medical history is found by his mother unresponsive next to an empty pill bottle. At the time of EMS arrival, the patient is noted to have agonal respirations and does not respond to painful stimuli. En route to the hospital, he is given 2mg of IN naloxone and has a generalized tonic-clonic seizure that terminates after 2mg of IV lorazepam is administered. He is then administered another 2 mg of IN naloxone and develops another generalized tonic-clonic seizure that stops after 4mg of IV lorazepam. His initial vital signs in the ED are: BP, 98/42 mm Hg; HR, 102 beats/minute; RR 20 breaths/minute; T, 98.1 degrees Fahrenheit (rectal); O2 Saturation, 95% (RA). The Poison Center is called for assistance.




2.     A 23-year-old healthy man presents to the ED for nausea, vomiting, abdominal pain, and blurry vision after an ingestion of an unknown liquid 11 hours ago. His initial vital signs are: BP, 180/98 mm Hg; HR, 80 beats/minute; RR, 18 breaths/minute; T, afebrile; O2 Sat, 99% (RA). His laboratory studies are notable for a venous blood gas: pH, 7.02, PCO2, 26 mm Hg, HCO3, 7 mEq/L; serum creatinine, 4.2 mg/dL.



3.     A 30-year-old man with a past medical history of HIV and major depression presents to the ED after intentional ingestion of his home medications. The patient had several witnessed generalized tonic-clonic seizures in the ED that were refractory to initial benzodiazepine administration and he is endotracheally intubated for airway protection. His initial vital signs are: BP, 110/50 mm/Hg; HR, 130 beats/minute; RR, intubated; T, 97.2 degrees Fahrenheit; O2 Sat, 100% (FiO2 100%).
