New York City Poison Center Consultants' Conference
June 2023

Consultants' Conference is a monthly meeting attended by medical toxicologists, emergency physicians, pharmacists and other learners from all over the Northeast. It is an educational activity in which the Consultants (renowned experts in various medical specialties) provide insight into the analysis and approach to the diagnosis and treatment of interesting toxicologic cases. The cases are presented mostly by the NYU Toxicology fellows in training, but anyone can present their case if deemed appropriate. Learners of all levels can benefit from the rich and often dynamic discourse.

The next NYC Poison Center Grand Rounds and Consultants' Conference will be held on June 1, 2023. Grand Rounds will start at 1PM EST and Consultants' Conference will follow immediately. The topic and speaker will be:

Fomepizole for Acetaminophen Toxicity - Friend or Foe?
 Brian Wiener, MD
Senior Fellow in Medical Toxicology
The Ronald O. Perelman Department of Emergency Medicine
NYU Grossman School of Medicine

Consultants' Conference will take place from 2 PM - 4 PM EST.

Cases to be discussed for Consultants' Conference include:

1. A 5-year-old girl with a past medical history of transfusion-dependent beta-thalassemia complicated by iron overload is admitted to the hospital for a regularly scheduled treatment. During her treatment, she receives an intravenous medication in one hour that is usually administered over 12 hours. Her vital signs after this error is recognized are: BP, 110/55 mmHg; HR, 110 beats/minute; RR, 18 breaths/minute; T, afebrile; O2 Sat, 96% (RA). She is asymptomatic.

2. A 57-year-old woman with a past medical history of major depression presents to the hospital via EMS after a suicide attempt where she ingested multiple pills and intentionally inhaled a vaporized laboratory grade liquid for two hours. She presents with a depressed mental status but has an otherwise unremarkable physical examination. Her initial vital signs are: BP, 88/57 mm Hg; HR, 49 beats/minute; RR, 10 breaths/minute; T, 93.2 degrees Fahrenheit; O2 Sat, 100% (RA). Her venous blood gas shows a carboxyhemoglobin level of 19%. The Poison Center is called for assistance.

3. An 81-year-old woman with a past medical history of atrial fibrillation and congestive heart failure presents to an ED with coffee-ground emesis and altered mental state for one day. Her initial vital signs in the ED are: BP, 175/116 mm Hg; HR, 64 beats/min; RR, 36 breaths/min; T, 37.3 degrees Celsius; O2 Sat, 97% (room air).
