[epicproviders] Effective 3/2/2026: Heart Failure Admission & Discharge Order Sets Updates and Core Measure Requirements

Attention Inpatient Providers:

Effective March 2, 2026: Changes have been made to the Admission order set for heart failure to decrease readmission and mortality rates, and to ensure compliance with new DNV accreditation requirements, thereby maintaining current adherence to evidence-based guidelines.

Order set names, layouts, and certain sections have been updated to reflect modern care for heart failure, including new mandatory items. All references to congestive heart failure (CHF) are now referred to as heart failure (HF).
