[LabUpdates] Laboratory Formulary Committee Updates - Blood Culture Molecular Testing

Medicine and laboratory testing are constantly changing. In an attempt to keep us all on the same page, the Laboratory Formulary Committee will be regularly sending out emails with recent updates and guidances. Below are the short versions of relevant changes. More in-depth resources about the changes are attached.

Blood culture molecular panel: Beginning December 6th, 2021, Microbiology will automatically perform a multiplex molecular assay on initial positive routine blood cultures (LAB462) for inpatients of all ages; collection and ordering remain unchanged. This molecular panel, an updated version of the one currently in use for pediatric patients, detects multiple bacteria, yeast, and antimicrobial resistance genes.

  *   This supplemental analysis will initially be performed daily on all 3 shifts for patients ≤18 years and during the day shift only for adults; 24/7 testing for patients of all ages will be implemented as soon as feasible.
  *   For pediatric patients: telephonic notification of the initial positive culture will include both Gram stain and molecular panel results.
  *   For adult patients: molecular panel results will be reported with the Gram stain report via telephone.  If not available at the time of the Gram stain, molecular panel results will be available for review in Epic (panel results may require up to 18 hours after the Gram stain positivity).
Standard identification and antimicrobial susceptibility testing of isolates will continue to be performed.
Infectious Diseases consultation is strongly recommended when S. aureus (MRSA or MSSA) or yeast are detected.
