Dear Upstate Nursing Team,
Many of us joined the Commission on Magnet® call, where we received truly exciting news: Upstate Nursing has earned our 2nd Magnet® redesignation! While some experienced audio issues on the call, I’m pleased to share that a full recording is available on our Nursing website. This achievement reflects the extraordinary work of every nurse across our organization—clinical, education, research, leadership, and support roles alike. Magnet® recognition affirms our commitment to excellence in patient care, professional practice, and continuous improvement.
>From the Commission’s Executive Summary, several exemplars were called out that showcase the depth of our practice and innovation:
Structural Empowerment (SE13b) — Culturally & Socially Sensitive Care (Ambulatory):
* Transition to Care Program: Interdisciplinary escalation rounds twice weekly to enable complex discharges; nurses and social workers align supports to cultural/social needs (e.g., coordinating travel and mobility for a long-stay TBI patient and confirming safe arrival).
* Inclusive Health Services: Central NY’s Designated AIDS Center delivering comprehensive case management, education, and affirming services; 442 patients received gender‑affirming care; 226 case‑managed; 97 engaged in therapy.
* Med‑to‑Bed Opt‑In: Providing a 30‑day supply of medications prior to discharge to reduce readmissions and support continuity.
* Food Insecurity Program: Nurses screen for social drivers of health; in six months, 600 food bags distributed (impacting 1,000+ patients) via the Nappi Wellness Institute pantry and monthly Mobile Food Bank.
* Additional examples: ASL education initiated by ED nursing for care of hearing‑impaired/deaf patients; culturally/religiously aligned meals on Adult Psychiatry; pediatric hair‑equity initiative ensuring inclusive hair‑care supplies.
Exemplary Professional Practice (EP20EOb) — Nurse‑Sensitive Ambulatory Quality Indicator:
* Surgical Errors: Organization‑wide ambulatory performance outperformed national benchmark and cohort in the majority of eight quarters across 100% of eligible ambulatory settings/units.
New Knowledge, Innovations, and Improvements (NK7) — Nursing‑Led Innovations:
* Pediatric Stroke Pathway: Multidisciplinary program, with nurses as key contributors, created diagnostic algorithms and treatment guidelines—enabling early diagnosis and treatment of eight pediatric stroke patients in 18 months.
* Virtual Reality (VR) in Burn Care: Nursing‑designed non‑pharmacologic pain strategy (grant: $15,000) reducing pain perception and improving tolerance and upper‑extremity range of motion during dressing changes.
* Hospital at Home: Ambulatory nurses and physicians delivering high‑quality in‑home care (community resources, home health, IV, imaging, labs), increasing bed capacity and reducing costs—recognized with LeadingAge New York “Innovation of the Year.”
