[EXTERNAL] *New* Testing Availability for SUNY Required Weekly Testing of Campus Faculty and Staff

Dear campus faculty and staff,

Starting the week of February 15, the following testing sites and processes will be added to expand availability of the pooled saliva testing process to meet the SUNY requirement of weekly testing of 100% of faculty, staff, and students on campus at Upstate Medical University.


  *   IHP Testing Site on Thursdays  - **Appointment Required**
Starting on Thursday, February 18th, there will be a weekly staffed testing site at IHP:

  *   Day: Thursdays
  *   Times: 11:30am-2:30pm
  *   Location: Classrooms A&B at IHP

  *   Self-Administered Testing at WSK and IHP - ** No Appointment Required**
For Upstate faculty, staff, and students that have previously participated in the pooled saliva testing, Upstate will be implementing a new Self-Administered testing process at Weiskotten and IHP.
**IMPORTANT: If you have not previously participated in the pooled saliva testing, you must first sign up to attend one of the staffed testing sessions where the testing team will walk you through the testing process.  After that initial test, you will be able to participate in the self-administered testing process going forward.   *   Self-Administered Testing details are as follows:
     *   Locations & Hours of Operation:
        *   Weiskotten Main Lobby
           *   Mondays - Thursdays from 8:30am - 3:00pm
           *   Fridays from 8:30am -12:30pm
        *   IHP Main Lobby
           *   Mondays - Thursdays from 6:30am - 3:30pm
           *   Fridays from 6:30am - 12:30pm
     *   Process: Upstate students, faculty, and staff that need to be tested would:
        *   Stop by the main lobby desk at each building during set business hours to receive a test kit from the Public Safety Officer.
        *   Show their Upstate ID badge to the Public Safety Officer.
        *   Fill out a form with required information before the Public Safety Officer can provide them with the test kit.
        *   Receive a test kit and biohazard bag from the Public Safety Officer.
        *   Go to a designated location near the main lobby desk at each building to complete the test or take the test kit with them to complete at their work station.
        *   Register the test kit in the web app.
           *   READ attached document titled Instructions-Registration App for COVID-19 Saliva Pool Testing
        *   Self-administer the test.
           *   READ attached document titled Instructions-Self-Administered Testing for COVID-19 Saliva Pool Testing
        *   Drop off the completed test kit in a biohazard bag at a designated secure location near the Public Safety Officer in the main lobby of each building.
        *   IMPORTANT: It is very important that all the steps be followed in the outlined process or else the test kit will not be able to be processed by the lab.
